Health Insurance Coverage

*** Insurance benefit coverage varies between plans and within plans. It is ultimately the client responsibility to understand their coverage and required copays/coinsurance and deductible. If you are unsure of your benefit details, please call the member number on your insurance card and ask the following questions:

 

  • Do I have coverage for procedure codes 97802 and 97803?

  • Do I need a referral and/or pre-authorization?

  • Are there restrictions to my coverage? If so, what conditions are covered?

  • What is my copay or coinsurance? Do I have a deductible, if so, how much is it and how much is left?

  • Do I have a restricted number of visits?

  • Blue Cross, Blue Shield

  • United Healthcare

  • Medicare - Covers diabetes and chronic kidney disease with referral

 

If your insurance company is not listed, there is a chance they will reimburse a portion of your out of network expenses. Contact your member services to inquire. A receipt with appropriate medical office visit codes will be provided.
